6 Conclusions and Recommendations
Resorting to the reasons and categorizing mistakes that resulted directly from the failure of the decision maker, Table 6.1 and Graph 6.1 indicate that over 80 percent of mistakes resulted from the failure of the decision maker to take some type of action.
The total number of mistakes was 33. A distribution attributed to each reason can be seen in Table 6.2 and Graph 6.1.
