An interesting, yet not surprising, fact is that you cannot rehearse lessons if you never learned them in the first place. Organizations initially have to decide to learn the lessons. They have to decide that they want to learn. Such a decision can be initiated by executive management, by middle-level management, or by the knowledge workers. Lesson learning can occur at one or all three of these levels. Yet the learning must commence to enable business improvement.
