Hazard 10: Confined Spaces, Including Caving and Mines
A company director was fined £30,000 after a worker was buried alive. It took the rescue team 36 hours to find him because the excavation was so deep. The upper part of the excavation collapsed because it was not supported. No method statement had been produced for this high risk activity.
