2021 Documentation Guideline Changes
In November 2019, CMS finalized their proposal to align E/M coding guidelines with the changes revised by the American Medical Association (AMA), CPT Editorial Panel for office/outpatient visits. This new foundation includes revisions to the E/M office visit code descriptors (99202–99215) and Prolonged Services (99354, 99355, 99356, 99XXX). Provisions for services furnished under the Medicare Physician Fee Schedule (PFS) are effective on or after January 1, 2021.
According to the CPT Editorial Panel, revisions were created to E/M office visits based on four primary objectives:
