Directions: Turn back to the survey that you completed on pages 1 and 2. Transfer your circled answers to the sheet below, making sure to match up your response for each question with the numbers in the four columns. Note that the numbers go from left to right. When you have finished, total up your score in each column and then transfer these totals to the line labeled “Raw Score” on the next page.
