This chapter describes a major initiative undertaken across Kaiser Permanente to redesign primary care practices in ways that could optimize the value of the electronic health record (EHR) while mitigating the new work demands associated with KP HealthConnect. The 21st Century Care Innovation Project proved effective in increasing various types of capacity in pilot sites, and many of the practice innovations that grew out of the initiative have been widely adopted.
