Attachment 15. DESIGN REVIEW APPROVAL FORM
The Design Review Approval Form in Figure A15-1 is used to document the completion of all the elements of a design review. While you may have documented each of the elements (i.e., Design Review Package, Design Review, etc.) individually, it is a good idea to have one form where all the elements are recognized and approved. This will come in handy when you are assembling all the data for sell-off.
