"Randy Wiggington"  should be "Randy Wigginton".

(91)  first line of last paragraph;
All of the spaces are missing from the first line of the paragraph, it reads:

(114)  1st paragraph;
"Roger van Oech"  
should read:
"Roger von Oech"


(122)  2nd line of 2nd paragraph;
"Haelerli" should be "Haeberli"


[140]  toward end of second paragraph;
"Bill" should be "Bob"
