The ebook version of the book is missing pages. Here is a comment copied directly from Amazon:

"From what I can read the book looks excellent. But, the ebook version is missing many pages. I've downloaded it twice and it misses the same pages both times."

This is in regards to the softcover "11-18-2022: 6th release".
Page "v" which apparently contains the table of contents for chapters 5 onward, is missing and instead page "5" is printed there, so the sequence is iv,5,vi. Page 5 also appears in its proper place so my copy has 2 page 5s and no page "v"

Could someone send me an image of page v, or post it on the errata page, so that I/others can can print it and stick it in the book?
